We offer only an on-line form (below) because it is faster, easier and less prone to misunderstanding than a traditional paper form

• The online form guides you through the questions and automatically provides supporting information when needed.
• As you proceed through the online form, it will use each response to automatically display further questions or to automatically remove questions which are not relevant to you.
• When you’ve completed filling out the online form, a copy is instantly emailed to the FVFS as well as to your specified email address. You immediately receive a copy for your records.
• You can save a partially filled out online form and complete it later.
• Payment can then be made by EFT, cheque or cash.
• If you prefer to lodge a form by post please complete the form on-line and print it. You can then either “Submit” the form automatically or post a paper copy to the FVFS.
